• Must live within a 50-mile radius of Itasca, IL.
• Must be at least 21 years of age.
• All adults in the household must be aware of and agree to the adoption.
• Written approval from your landlord is required if you rent.
• A valid ID with your current address is needed (proof of address if ID is not current).
• You must be approved as a suitable adoptive home.
